1. RFP for the Adaptive Modifications Program – For Construction

This Request for Proposals (“RFP”) provides interested contractors with the information required to prepare and submit proposals for consideration by the Philadelphia Housing Development Corporation (“PHDC”) to satisfy the need for construction services to be provided to the homes of low income, physically disabled residents of Philadelphia, which will be paid for by grants under the Adaptive Modifications Program (AMP).

2. RFP for the Adaptive Modifications Program – For Durable Medical Equipment

 This Request for Proposals (“RFP”) provides interested contractors with the information required to prepare and submit proposals for consideration by the Philadelphia Housing Development Corporation (“PHDC”) to satisfy the need for durable medical equipment (“DME”) to be provided to the homes of physically disabled residents of Philadelphia which will be paid for by grants under the Adaptive Modifications Program (AMP).

3. RFP for the Adaptive Modifications Program – For Occupational Therapy Consultations

 This Request for Proposals (“RFP”) provides interested contractors with the information required to prepare and submit proposals for consideration by the Philadelphia Housing Development Corporation (“PHDC”) to satisfy the need for occupational therapy consultations (“OTC”) to be provided in the homes of physically disabled residents of Philadelphia, which will be paid for by grants under the Adaptive Modifications Program (“AMP”).

The following applies to all three AMP RFPs:

Recipients of AMP grants will be the customers of the contractors selected to participate in the program. Contracts for reimbursement will be offered for all or part of the period of July 1, 2018 to June 30, 2019.  At the sole discretion of PHDC, the contract may be renewed for an additional one-year period to have continuing services for up to two years in total.

Pre-Proposal Briefing: Wednesday, May 2, 2018 at 9:00 am
Contractors desiring to participate in AMP are strongly urged to attend the RFP Briefing Session on Wednesday, May 2, 2018 at 9:00 A.M. in the 3rd Floor Auditorium at 1234 Market Street, Philadelphia, PA. Topics covered at the briefing will include:

  • State of the Program and expected funding levels
  • Review of the RFP package, including insurance and indemnification requirements and critical documents for contractors to include in a response
  • Contract preparation steps, including prequalification as a PHDC contractor

Contractors are advised that copies of this Request for Proposals will not be available at the Pre-Proposal Briefing.

Proposal Deadline: by 12:00 pm (Noon) on Friday, May 11, 2018

Please review the RFP package and bring your questions to the pre-proposal briefing. Contractors may call with questions or arrange appointments with AMP staff to resolve RFP response preparation questions.  Appointments will be entertained from May 2, 2018 (after the briefing) to May 11, 2018 (before the due date).  Contractors should schedule appointments as early as possible as PHDC staff may have limited time to meet with contractors during this period.  Contact names and telephone numbers will be provided at the Briefing.  This offer of individual appointments is only available to those contractors who attend the Briefing.
